If you are a HMOI/Blue Advantage HMO member or an Aetna EMHC Employee EPO member, please feel free to print out the appropriate PCP selection form and mail or fax the completed form to Elmcare Member Services.

If you are a Humana HMO member please refer to the back of your identification card for your Health Plan's customer service line. You must contact your Health Plan directly to choose or change your PCP.

Emergency Room Services

Remember: Obtain your Emergency Room care at Elmhurst Memorial Hospital where your Primary Care Physician is on staff. If it is a life-threatening emergency go to the closest Emergency Room.
